Concordia Dental Plan


United Concordia dental plan products offer employers and individuals a wide variety of flexible benefit options to meet their unique needs at affordable rates. Concordia plan holders also have access to the provider's Advantage Plus network of over 111,000 dentists practicing all over the US.

For employers, Concordia dental plan options include:

Concordia Flex is an employer-funded passive PPO plan that is available for businesses with as little as two employees. Employers can avail of over 9,000 program options with further customization available for organizations with more than 200 enrolled employees. Concordia Preferred is an employer-funded active PPO plan that offers covered individuals the freedom to see any dental care provider, but will reward them with significant cost savings when they use an in-network dentist. Concordia Plus is a dental HMO plan that requires covered individuals to nominate a primary dental care provider who will coordinate all their care. The plan covers all eligible preventive and diagnostic services with little or no copayment and no annual maximums and no deductibles. Concordia Choice is a PPO plan that offers a wide variety of affordable and flexible program options as well as low minimum participation requirements. Discounts on eye care services are also offered through Davis Vision. For individuals, Concordia dental plan options include: IND 100 offers 100% coverage for preventive and diagnostic dental services but does not cover basic and major services. It has no annual deductible and no annual maximum per insured person. IND 200 has similar benefits as IND 100 but charges an annual deductible of $50 per insured person and an annual maximum of $2,000. IND 300 covers diagnostic and preventive services, as well as basic services, for up to 50% but does not cover major services. There is an annual deductible of $50 per insured individual and annual maximum of $1,500. IND 400 offers 100% coverage for diagnostic services, 80% for basic services and 50% for selected basic services and major services. A $50 annual deductible is charged per insured individual and $2,000 annual maximum. IND 500 charges the same deductible and has the same annual maximum as IND 400 but offers 60% coverage on selected basic services and 60% on major services (after a six-month waiting period). Diagnostic and preventive services, however, are not covered.
